I checked in a big series of changes for :ref:`presto`.

Especially the new plugin :mod:`lino_xl.lib.orders`.

I moved PriceRule and PriceFactors from :mod:`lino_tera.lib.courses`
to :mod:`lino_xl.lib.products`.
