Continued to work on 
the new :class:`CustomTable <lino.core.table.CustomTable>` 
class.
Added `column_defaults` to specify a default column width.
:class:`OverviewClientsByUser 
<lino.apps.dsbe.models.OverviewClientsByUser>` 
now almost has a total row.
